The quote from bustacell spells out the insane combo kill rewards (basically moving combos). For every moving combo kill you get a Holy Shit but for every 5th one you get one of the five reward levels.
Combo Whore, King, and Maniac are rewards for the total count of combo kills (both moving and standing combos). They are set at 20, 40, and 60.
All of these rewards were originally created by Sp0ngeb0b for the UrS server. Some of the sounds are built-in to UT99 but I think a couple may have come from UT3, can't remember the specifics of this at the moment.
Additionally, all of the rewards are set to repeat if the maximums are ever reached during a match.
